commcare.health is a home care company pioneered by doctors who are passionate about caring for older persons.
Why we created commcare.health
As doctors, we noticed a trend with our elderly patients at the clinic. As they became more frail, initially they came in to see their GP more often. However, they often became house bound. At this point, they stopped coming completely. Their care would deteriorate, and their progress to being placed in an residential aged care facility, commonly after a precipitous clinical event, was abrupt and expedited.
From that observation, our mission was laid out – to integrate health care with home care, allowing older persons to live independently at home for as long as possible.
